Requirements:
Must have: Birth Certificate, Homeless Verification Card, Proof of Income, SS Card, Photo ID
Engagement with case manager is required ever 60 days.
Offerings:
Helps with short-term rental assistance, case management, transportation, and food assistance for families, single individuals and TAY households. Offers housing specialist services to help maintain housing and increase income, supporing self sufficiency.
